Because the position is rotated between the top medical personnel of the Navy, Air Force and Army. The Navy equivalent to a General is an Admiral.

...cardiothoracic surgeons , and vascular surgeons are all at the top end of the list. Specialized surgeons , depending on the 'specialty', often make much more than a general surgeon , but this is certainly not always the case.
