That's a tough decision. If you want to manage an empire, get Civilization . If you want to manage a family, get Sims. If you want to do both (kind of), get Sims Medieval.

CIV V has more Content, but Shogun 2 is better in every other regard (graphics, online, gameplay), depends if you like turn based or RTS games.
