It seems easy enough to do, apart from the MySQL Connector which you will need to download and install yourself. The rest is simple enough, you just create a function, let it take the params you need and do the connection, query and results in the function...

DO you already have an Access database set up? you need to load the data from the Access Table into the combobox... If you have an Access Database with tables on it... I'll come back to you
